If your loved one is receiving home care from Vytality At Home, you can stay informed of every aspect of your loved one’s care through our Family Portal. Simply log in to our secure and easy-to-use platform on any device with internet access, for real-time access to care information, notes, schedules, invoices, and more.

Easy to stay connected

Our Family Portal is designed to help you stay in touch with all aspects of your loved one’s home care. Family members can be allocated their own log-in details (just contact the Vytality office for details). So there’s no need for emails or long phone calls to bring everyone involved up to date on when the caregiver arrived, or when their next visit is, for example.

Family Portal: Benefits not Features

It’s all too easy to read a whole list of features for tech and software, but not relate it to what you might actually need the for! So we’ve broken down the key features of the Family Portal and show how they can help you and your family monitor and manage your loved one’s care whether you live here in Calgary, other provinces in Canada, or indeed anywhere in the world!

Our Vytality at Home mobile app accesses the same Portal platform information, so you can stay connected and manage care-related tasks on the go, for flexibility and convenience. Since the Portal can be accessed at anytime 24/7/365, this is a really useful feature for those living abroad or working night shifts, etc.

• Real-Time Clock-In and Clock-Out

Every time a caregiver comes to visit, they will log in, and then log out when they leave. So you can view these live time-stamped updates, and be reassured that a caregiver has arrived on time, daytime or evening, sunshine or snow.

• Daily Shift Summaries

If there is one thing better than knowing when a caregiver arrived, it’s knowing what they did during their 3+hour visit with your loved one. After every visit, your caregiver will create notes detailing what tasks they completed, and also how your loved one is doing. They will also note any changes they saw or observations they made, to provide a comprehensive timeline of incremental changes that might otherwise not get noted.

This is also extremely useful if your loved one has some element of short term memory loss and has problems recalling exactly what happens each day. One of our social team uses it to find out what happened during the caregiver’s visit. She reads the notes online before she calls her mum. So if her mum says the caregiver didn’t arrive, or she couldn’t go outside, or didn’t get a full wash, she will have already checked the notes and saw that in actual fact, all those tasks were done. It’s just that her mum cannot always remember.

• Track Tasks and Completion

All our caregivers have a full list of scheduled tasks for each home care visit they make to your loved one. These tasks align with your loved one’s comprehensive care plan, which was created after their initial assessment, and reviewed and updated as required. As a family member, you can log in and see exactly what was completed on each and every visit. The caregiver will include notes for you in the Portal if something wasn’t done for any reason, and explain why. (You can always call the office to discuss these if you wish.)

• See All Scheduled Visits

Many families create family calendars which they share between them to help with planning events, including holidays and visits to see their loved ones. Some want to be there when the caregiver arrives to have a chat, whilst others prefer to keep their visits separate. The Vytality at Home Family Portal includes a full visit calendar, so you can keep track of upcoming caregiver visits, changes to the schedule, or new appointments, and plan accordingly.

• Meet the Care Team

Our Vytality at Home Family Portal gives you details of the caregivers who are supporting your loved one, including their roles and experience. We carefully screen all our caregivers to ensure they have the skills and personality to provide the care required and also share our core values. All our caregivers undergo a a full vulnerable sector / background check, and actively participate in orientation, training and a competency exam. We are very picky as to the caregivers we employ and invest much time and effort into their professional wellbeing.

• Billing and Invoicing

As you would expect, you can access all your billing details, past invoices, and payment information whenever you need it. You can process payment directly through the Portal too, making it easier for family members to have total transparency and convenience of who has paid what, and when.
